Most of the outages in the US happen in the distribution network; system wide outages caused by a collapse of the transmission/generation system are a lot less frequent.
The weather events that cause big distribution outages are hurricanes, freezing rain, and more recently fires.
Every year, the US has weather events where more than 1 million customers lose power. There are events causing several million customers to lose power. Whenever you have a big weather event, some customers lose power for up to 2/3 weeks.
